Ultimately, a remarkable Clinical Nurse Case Manager Lead will establish warm and meaningful ties with each patient their families, and bring together a cohesive, efficient, and effective case management team. Case management activities incorporate a collaborative, interdisciplinary team approach. The candidate can quickly identify and remove burdens from patients to ensure they are afforded the time and space to focus on their health. The nurse case manager lead will participate in and lead a team to review patient charts, meet with providers, patients and their families to establish the most suitable course of action.
